I instruct Cornish Cousins to undertake on my behalf the following project:

Family History Research based on a fee of £15.00 per hour

I understand that any GRO Certificates will be charged at cost price and

travelling and postage and packing will be additional.

Forward Searching based on a fee of £20.00 per hour

(up to a limit of _________ hours).

I understand that any GRO Certificates will be charged at cost price and

travelling and postage and packing will be additional.

Production of a Family Tree

based on the information which I supply at a fee of £1.00 per name.

(Postage & packing is included with the first tree. Any additional copies will

be charged at 20% of the original price, plus postage & packing.)

Organise a Family Reunion Get-together with my family's assistance

for an 'up front fee of £100.00.

Additional expenses of postage, travelling and telephone will be charged, together

with a commission of £1.00 per ticket sale.

(Any expenses of the event, eg: venue, catering, entertainment, photographer, etc.

are the responsibility of the family who have instructed the event. Cornish Cousins

suggest that tickets are sold to meet these expenses.)
